Help us make the most of your visit and time with Bunbury ENT by completing the patient information and health questionnaire forms below prior to your visit here. To enable us to book an appointment for you please ensure your GP has sent a referral through to our practice at [email protected]

Bunbury ENT is a private practice, and does not offer bulk-billing. Payment is required at the time of the treatment. The fee for an appointment is outlined to patients at the time of booking.

You will receive an email and sms once your appointment is booked.

We process Medicare rebates on the day at the time of payment. Patients eligible for a Medicare rebate will receive their rebate into the bank account linked to their Medicare within 1-2 business days.

A 24-hour cancellation policy applies for appointments at Bunbury ENT.

Please provide us with no less than 24-hours notice if you need to cancel/change your appointment. For any appointments cancelled within 24-hours, we require a flat prepayment of $250 if you wish to rebook with us. All prepayment fees must be paid prior to booking your appointment.

Health Information Collection and Use Consent Form

As a patient of our medical practice, we require you to provide us with your personal details and a full medical history, so that we may properly assess, diagnose, treat and be proactive in your health care needs.

We aim to protect the privacy and secure storage of your health information. You can request a copy of our privacy policy, which includes information about the collection, use and disclosure of your health information.

We require your consent to collect personal information about you and to use the information you provide in the following ways:

  • Administrative purposes in running our medical practice.
  • Billing purposes, including compliance with Medicare and Health Insurance Commission requirements.
  • Disclosure to others involved in your healthcare including treating doctors and specialists outside this medical practice. This may occur though referral to other doctors, or for medical tests and in the reports or results returned to us following referrals.
  • Disclosure to other doctors in the practice, locums etc. attached to the practice for the purpose of patient care and teaching.
  • For research and quality assurance activities to improve individual and community health care and practice management. Usually, information that does not identify you is used but, should information that will identify you be required, you will be informed and given the opportunity to “opt out” of any involvement.
  • For reminder letters/sms which may be sent to you regarding your health care and management. You can decline to have your health information used in all or some of the ways outlined above but it may influence our ability to manage your health care to provide the best outcome for you.

I understand, as a patient of this medical practice, Bunbury ENT is a private practice and each appointment will incur a fee which is required to be paid on the day of my consultation and where required, knowingly accept Bunbury ENT will submit my claim to Medicare where applicable (provided I have a valid referral on file). Should I have any queries or concerns, I am aware I can raise my concern with staff at any point.
